Responsibilities

• Client needs assistance ambulating, hoyer lift available.
• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs, patient behavior, and overall condition; report changes promptly to healthcare staff
• Medication reminders following proper protocols and HIPAA regulations to ensure patient safety
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
• Operate patient transfer equipment such as Hoyer lifts and ensure safe manual handling during transfers, mobility safety.
• Perform light housekeeping including but not limited to: laundry, dishes, cleaning rooms used by client, sweeping/mopping, taking out the trash.
• Assist with transportation for appointments or community outings
• Follow all safety procedures including first aid, CPR certification requirements, and emergency response protocols
• Document observations accurately in care records while respecting residents’ privacy and confidentiality

Requirements

• Hours requested: EOW & EO 2 weekdays 9a-5p (bedbound) Mon-Fri 10am-4pm (fall risk)
• Prior experience a must!
• Must have knowledge of home health environments or worked in assisted living facilities, nursing homes
• Knowledge of patient observation techniques, medication reminders, and vital signs monitoring
• Ability to assist with lifting up to 50 lbs and manual handling safely using appropriate techniques and equipment such as Hoyer lifts
• Familiarity with HIPAA regulations to protect patient privacy at all times
• Strong communication skills for effective client interaction and teamwork within healthcare settings
