Registered Nurse (RN) - Hospice

RN Case ManagerTypical schedule is Monday to Friday, 8 AM to 5 PM, with on-call and weekend rotation.Guaranteed hours: 40. Contract length: 13 weeks.Current RN license in the stateValid driver's license and reliable transportation2 years of recent acute care, home care, or hospice experience preferredExperience with clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferredFamiliarity with HCHB is strongly preferredProvide skilled nursing and coordinate care to patients and their families in a residential setting.Observe, assess, evaluate, and plan interventions in relation to patients' physical and psychosocial statuses.Oversee care provided by Home Health Aides and CNAs; evaluate compliance with the plan of care.Ensure continuity of care through effective communication and case conferences with interdisciplinary teams.Provide quality care and appropriate symptom management to promote optimal health and quality of life.Maintain accurate and complete patient records in accordance with agency standards.Participate in quality improvement activities and in-service training as assigned.
