Responsibilities
- Perform comprehensive occupational therapy evaluations for residents in the SNF setting.
- Develop and implement individualized treatment plans targeting functional mobility, activities of daily living, cognitive goals, and safety.
- Deliver evidence-based therapies, monitor patient progress, and adjust plans to meet goals.
- Document assessments, progress notes, and discharge planning in accordance with facility policies.
- Collaborate with nursing, therapy, social work, and other members of the multidisciplinary team to optimize care and transitions of care.
- Educate residents and families on home exercise programs, fall prevention, and adaptive strategies to enhance independence.

Requirements
- Active Occupational Therapist license in California or eligibility to obtain licensure at start.
- Experience in acute care or skilled nursing facility settings preferred.
- Strong assessment, treatment planning, and documentation skills.
- Ability to work 35 hours per week within an 08:00–16:30 schedule.
- Excellent communication and teamwork skills to support a collaborative care approach.
- Must be able to travel to Alameda, CA and adapt to a SNF environment.
