LPN Continuing Care

As a continuing care LPN, providing top-notch patient care is paramount. Under the direction of an RN, mid-level provider, or physician, you will be responsible for assessing, planning, implementing, and evaluating patient care. You will be a vital part of the team, making a real difference in the lives of patients and their families. You will collect and monitor assessment data, reporting physiological, psychological, sociocultural, and spiritual needs of patients and families. You will provide direct patient care consistent with policies and procedures, and unit-based routines. You will inform patients and families about treatments and medications, equipment being utilized, and other related activities. You will document care provided via computer or written patient records in accordance with guidelines. You will communicate with patients and families in a caring and respectful manner. You will collaborate with other members of the healthcare team to ensure continuity of care and coordination of service. You will clean the patient's environment and equipment, and communicate any malfunctions to the appropriate personnel. You will document patient chargeable items according to standards. You will participate in selected quality monitoring activities under the direction of the RN or mid-level provider. You will demonstrate a high standard of moral and ethical behavior. You will recognize ethical behaviors and identify resources for resolution as needed. You will advocate for privacy, confidentiality, and security of patient and staff data. You will actively participate in safety initiatives and complete all position-related safety competencies on a timely basis. You may be assigned other duties as needed.

Experience in a skilled nursing facility setting

Weekly Pay: $1472 - $1549
