Job Summary

As a Nurse Practitioner (NP) or Physician Assistant (PA), you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities.

About the Role

We are seeking an experienced NP or PA with a background in Geriatrics or Internal Medicine to join our team. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player. You will work closely with our clinical directors, regional managers, and supervisors to provide exceptional care and support.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with our clinical team to ensure seamless care coordination

• Participate in quality improvement initiatives to reduce hospitalizations and improve patient outcomes

• Maintain accurate and timely documentation of patient care

Requirements

• Active NP or PA license (in the state of Pennsylvania)

• 1+ year of experience in:

  • Geriatric care

  • Documentation review

• Excellent communication, collaboration, and teamwork skills
