Ambulatory Care Pharmacist

- might need to fulfill limited inpatient staffing needs on a monthly basis JOB SUMMARY
- The Ambulatory Care Pharmacist is responsible for providing direct patient care through medication management and disease state monitoring in outpatient settings. The role works collaboratively within a multidisciplinary care team, partnering with physicians, advanced practice providers, nurses, and patients to optimize therapeutic outcomes, ensure medication safety and effectiveness, and support the achievement of patient care goals.
- JOB REQUIREMENTS
- Experience:
- Five years of clinical pharmacy practice experience in health system or ambulatory care settings.
- Prior or current experience as an ambulatory care pharmacist, with a demonstrated track record of achieving positive patient outcomes through effective medication therapy management preferred.
- Education:
- Pharm D - Doctoral of Pharmacy from an accredited pharmacy program.
- Strong knowledge of pharmacotherapy and evidence- based medicine.
- Completion of a PGY1 and/or PGY2 residency in Ambulatory Care is preferred
- Licensure & Certifications:
- MAPHAR - Licensed MA Pharmacist or eligible for reciprocal licensure at time of hire.
- BCPACP - Board of Pharmacy Specialtes in Pharmacotherapy or Board of Pharmacy Specialties in Ambulatory Care Pharmacy.
- Experience:
- SKILLS & ABILITIES
- Ability to work collaboratively within multidisciplinary teams.
- Excellent patient-care, written and verbal communication, attention to detail, and documentation skills.
- Excellent analytic and problem-solving skills.
- Leadership skills.
- Detail oriented, yet also understands organizational concepts.
- Able to organize, prioritize and manage multiple tasks simultaneously.
