School-Based Health Clinical Manager [Nurse Practitioner or Physician Assistant]

Description Location: Mansfield, Ohio Department: Medical Schedule: Full-Time | Onsite Status: Exempt | Credentialed Position Make a Difference Where Healthcare Meets the Community Third Street Family Health Services is looking for an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to serve as the School-Based Health Clinical Manager for our established School-Based Health (SBH) program. This is a unique opportunity for a provider who enjoys both patient care and leadership and is comfortable working independently while supporting a multidisciplinary team serving children, adolescents, and families in a school-based environment. The Clinical Manager provides day-to-day clinical and operational leadership for the SBH program while maintaining an appropriate patient care schedule. This role works closely with school partners, Third Street clinical leaders, behavioral health, vision, community health workers/case management, and administrative leadership to ensure students have access to coordinated, high-quality care. This position is best suited for an experienced provider. Because this provider will serve as the primary clinical provider at the site, we are seeking someone who is confident practicing within their scope, making clinical decisions, working with pediatric and adolescent patients, and functioning with a high degree of independence. What You'll Do Provide Patient Care Provide primary care to students and other patients within your licensure, credentialing, privileges, and scope of practice. Provide age-appropriate, student-centered care with a focus on children, adolescents, and families. Promote accessible, coordinated, equitable, and high-quality healthcare. Maintain timely clinical documentation, encounter closure, inbox management, and other provider responsibilities. Participate in quality improvement, peer review, audits, and other clinical quality activities. Serve as a clinical resource and mentor to SBH team members. Lead the School-Based Health Program Provide day-to-day leadership and oversight of the established School-Based Health program. Directly supervise assigned SBH staff and provide coaching, feedback, performance management, and professional development. Coordinate multidisciplinary team activities and facilitate effective team communication. Establish expectations for teamwork, communication, service delivery, patient experience, and accountability. Identify operational barriers and partner with organizational leadership to resolve them. Support hiring, onboarding, training, orientation, and competency development of assigned staff. Support Quality & Operations Help ensure consistent workflows for scheduling, patient flow, documentation, care coordination, quality, safety, and service delivery. Monitor clinical and operational performance and use available data to identify trends and opportunities for improvement. Partner with administrative and clinical leaders regarding staffing, facilities, technology, supplies, scheduling, and other operational needs. Ensure SBH operations align with Third Street policies and applicable HIPAA, FERPA, HRSA, clinical, regulatory, and school-based health requirements. Build School & Community Partnerships Serve as a clinical and operational liaison between Third Street and participating schools. Build positive working relationships with school administrators, teachers and staff, students, families, and community partners. Help identify and address barriers to student access, engagement, and care coordination. Participate in school and community outreach, health education, wellness activities, and other program activities as appropriate. Requirements What We're Looking For Required Current Ohio license as a Nurse Practitioner or Physician Assistant. Board certification appropriate to professional discipline. Current certifications, credentials, and clinical privileges required for the position. Valid driver's license. Experience providing primary care or other relevant clinical services. Experience working in a multidisciplinary healthcare environment. Strong clinical judgment and communication skills. Ability to work independently and balance direct patient care with administrative and leadership responsibilities. Demonstrated ability to supervise, coach, and develop staff. Knowledge of primary care, preventive health, integrated healthcare delivery, and care coordination. Knowledge of applicable healthcare privacy, regulatory, and compliance requirements, including HIPAA and FERPA. For Physician Assistant applicants: Completion of required clinical training/hours and eligibility to practice without an ongoing onsite physician training requirement. Preferred Experience providing care to children and adolescents. Experience working with families and school-age populations. Previous school-based health experience. Supervisory or healthcare leadership experience. Experience in an FQHC or community health center. Experience with quality improvement, population health, or care coordination. Experience working with multidisciplinary teams and community partners. Master's degree or other advanced education appropriate to the individual's professional discipline. Why School-Based Health?
