Social Worker

*NO VACCINE MANDATE* Elcor is recruiting for a full‑time Social Worker. Job duties include providing social services for residents, acting as a liaison between medical staff, families, and outside agencies, interviewing residents and families for social histories, ensuring that social and personal aspects of resident care are properly managed, collecting and updating information on community health and welfare resources, documenting care plans, assisting with admissions and discharges, conducting care plan & discharge meetings, and additional duties as assigned. Bachelor's degree in social work or closely related field required and experience in long‑term care preferred, familiarity with Point Click Care preferred.
