RN Case Manager Hospice

Triniti Home Health & Hospice is hiring an experienced hospice Registered Nurse (RN) in the role of a RNCase Manager Hospice in Princeton, MN . The ideal RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care. In this role, you will collaboratively works alongside an interdisciplinaryteam to provide end-of-life care to those needing hospice services in their home or community setting. You will provide individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care. Position Type: Full-time, benefits eligible position Wage Range: $42 - $47 / hour depending on experience Location: Princeton, Minnesota RN Case Manager Responsibilities: Succinctly convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs Anticipate, prevent and treat undesirable symptoms or secondary symptoms Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient Train, instruct and support the patient, family and caregiver Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner Consult and collaborate respectfully with the interdisciplinary team and others involved in the patient’s care Prepare and present all needed information for bi-weekly IDG meetings RN Case Manager Qualifications: Graduate of accredited school of nursing Current state of Minnesota RN licensure Three years recent nursing experience in acute, long-term care or home care/hospice/public health Patient care planning and patient teaching skills Critical thinker with excellent written and verbal communication skills Current Minnesota driver’s license, insurance and daily access to reliable vehicle Cassia Benefits: Competitive Pay with experience-based raises Tuition Assistance & Student Loan Forgiveness (site-specific) Paid Time Off (PTO) & 403(b)/401(k) with Employer Match Comprehensive Health Benefits (Medical, Dental, Vision, Disability, Life Insurance) for Full-Time Employees Employee Assistance Program with free confidential counseling/coaching for self and family members Employee Discount Program Pet Insurance Mentoring program & opportunities to grow your career About Us: Triniti Home Health & Hospice is a great environment to grow your career! we have a work hard, play hard mentality.
