Your typical day will include traveling to multiple patient homes within Watkinsville and nearby communities, reviewing clinical information and orders, completing visits, and documenting in an electronic medical record system. Caseloads and visit volumes are designed to allow thorough assessments and meaningful patient interactions, with an emphasis on quality outcomes, safety, and patient satisfaction. You will collaborate closely with an interdisciplinary team that may include physicians, therapists, social workers, and home health aides, coordinating services and communicating changes in patient status promptly.
