Date Posted: Jun 24, 2026 Application Deadline: 09/22/2026 Location: Richmond, TX – Richmond State Supported Living Center, 2100 Preston, Richmond Functional Title: Investigator IV Job Title: Investigator IV Agency: Texas Health and Human Services Commission (HHSC) Department: Incident Management Posting Number: 18326 Salary Range: $4,263.16 – $6,779.25 per month (Salary Group TEXAS-B-20) Shift: Day; Additional Shift: Days (First) Telework: Not Eligible Travel: Up to 10% Full Time: Yes Position Type: Regular, Non‑Exempt Responsibilities Serve as the SSLC’s Lead Investigator, coordinating investigations of unusual incidents, serious injuries, and allegations of resident abuse, neglect or exploitation (ANE). Follow incident‑management protocols, coordinate immediate actions, and report to Regulatory, HHSC State Office, and other agencies within required time frames. Conduct investigations: gather evidence, review video surveillance, interview witnesses and complainants, and collaborate with nursing, behavioral health, and habilitation staff. Evaluate, analyze, and summarize investigative findings; prepare concise investigative reports and present findings to supervisory staff. Provide oversight, coaching, on‑the‑job training, and mentoring to entry‑level investigative staff; develop and update investigative methods. Act as backup for the Incident Management Coordinator (IMC) or Director of Incident and Risk Management (DIRM); support coverage and business continuity. Participate in daily meetings, critical incident team (CIT) reviews, root‑cause analysis, and other committees; recommend improvements. Maintain primary contact relationships with external entities (Regulatory, law enforcement, OIG) to coordinate cases and ensure compliance with statutory standards. Track data for management reports, including trend analysis and high‑profile incident tracking; ensure accurate data entry and follow‑up. Assist with audits, inspections, and environmental or behavioral checks; provide feedback on procedures and practices. Provide support during emergency investigations on “on call” or rotating schedules; assist with disaster response and continuity of operations as needed. Essential Job Functions Attend work on a regular basis; work specific shift schedule and occasional rotating or overtime shifts per agency policy. Ensure rapid response to incident notifications, including contacting the on‑call administrator and initiating preliminary investigations. Review and approve investigative reports for compliance and accuracy; coach investigators on report writing. Coordinate data entry and maintain records in Unusual Incident Reporting (UIR) and other tracking systems. Serve on incident‑management and audit teams; provide input on policy, procedure, and regulatory compliance. Forward necessary information to external investigators and follow up on regulatory guidance and documentation. Knowledge, Skills, and Abilities Knowledge of state and federal laws, ICF‑IID regulations, and policies related to State Supported Living Centers. Knowledge of investigative principles, techniques, and procedures. Computer skills – Word, Excel, Outlook, and electronic health/incident management systems. Interviewing and fact‑gathering skills; ability to search records and retrieve relevant information. Formulation of comprehensive investigative reports; skill in critical thinking, problem solving, and research. Effective written and oral communication; ability to convey findings and testify in hearings or court proceedings. Ability to manage emotions, collaborate with multidisciplinary staff, and direct assistance from other team members. Leadership and mentoring skills to train and supervise coworkers; capacity to develop investigative techniques and best practices. Required Qualifications High school diploma or equivalent; four‑year college or university degree is preferred. Experience planning, coordinating, leading, or conducting investigations; quality assurance review experience is an asset. Experience writing detailed investigative reports. Minimum US citizenship or legal work status; successful background check, drug screening, and registry checks required. Ability to work weekends, holidays, overtime, and on call or rotating schedules. Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS) and HHS List of Excluded Individuals/Entities (LEIE).
