RN Nurse Navigator - Urology Ft Thomas

Job Type: Regular Scheduled Hours: 40 Job Summary: Reports to the Operations Manager, the RN Nurse Navigator functions as a member of the multidisciplinary team of care providers who reflect a culture of cooperation, enthusiasm, and respect. The Navigator is responsible for coordinating the appropriate care of the patient based on an evaluation/assessment of the patient (telephonically or in‑person) that matches the patients’ desires to the most applicable program that the Surgery Department offers. This includes education, continuity of care and support through all phases of diagnosis, treatment, and follow‑up care for patients, ensuring they maintain a high level of engagement and participation in the program that is critical to their success and the goals that they desire. The role is mainly clinical, but also incorporates business development and a high level of patient engagement skills. Job Description: Benefits: No Nights, Holidays, or Weekends. Current registration to practice nursing in the state of Kentucky. Obtains required CEUs, including all state required courses to maintain license. BLS Active Kentucky RN licensure Education: Graduate from an accredited nursing program Years of Experience: Required: 3+ years of RN experience Duties and Responsibilities: Conduct patient assessments (telephonically or in person) to aid patients in navigating and selecting the appropriate Urology care pathway. Provide a welcoming encounter (via phone or in person) for the patient, reflecting ease of use for accessing Urology or Urogynecology locations and timely education about specialty offerings. Assess patients clinically, using gathered information, protocols, and recommendations to guide patients toward the care pathway that best suits their needs. Work with other staff and support teams to guide patients through their chosen care pathway, ensuring they are informed and actively engaged; perform minor outpatient clinical duties as directed by the physician within licensure scope. Coordinate with providers, outpatient departments, hospital departments, and other support staff to ensure patients are engaged, compliant, and well connected across departments. Collaborate with all Surgical Division programs to maintain patient preferences and unique needs, transitioning patients to better‑suited program offerings when appropriate. Participate in meetings and coordinate with teams within the Surgical Division to improve patient care, satisfaction, and clinical outcomes aligned with department goals. Remain current with research and treatment options. Conduct follow‑up calls post‑discharge to assist patients with instructions, continuity of care, results, and to reduce readmission; seek patient feedback for program improvement. Serve as an educational resource for staff and assist in developing multidisciplinary training programs. Contribute to ongoing quality improvement plans and demonstrate willingness to participate. Track patient outcomes, satisfaction, and financial data to support programs, processes, and improvement activities; report key statistics as needed. Other duties as assigned. Required Skills and Knowledge: In‑depth knowledge of surgical and other available options for patients, using clinical assessment (telephonically) to determine the best course of action. Knowledge of community resources; proficient use of office equipment (computer, keyboard, printer, fax, etc.). Excellent communication skills; computer literate; ability to work efficiently in the EPIC EMR system. Manage a clinic schedule, perform physical exams, interpret lab results, prescribe and manage medication, complete pre‑operative evaluations, and ensure patients meet insurance requirements for surgical services. Navigate communication barriers and foster collaboration among patients, families, disciplines, departments, and community agencies. Demonstrated ability to work successfully in a team‑based, decision‑making culture. Demonstrated ability to work independently, achieving effective outcomes and on‑time performance. Experience in planning and coordinating multidisciplinary communication strategies, strategic initiatives, and events. Respond promptly to customer requests. Work carefully and precisely with attention to detail. Utilize resources wisely. Perform duties willingly and with initiative; share necessary information with co‑workers. Cooperate with other departments and work groups.
