Internal Medicine Provider Salary: $200,000+/Year based on experience LaSante Health Center is seeking an Internal Medicine Provider to join our multispecialty, community-based health center in Brooklyn. The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred. Responsibilities Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management Diagnose and treat acute and chronic medical conditions Collaborate with multidisciplinary teams to coordinate patient-centered care Counsel patients on lifestyle modification, disease prevention, and medication management Maintain accurate, timely documentation in the electronic health record (EHR) Participate in quality improvement initiatives and team meetings Complete additional tasks as directed by supervisor Qualifications Medical degree (MD, DO, NP, or PA) from an accredited institution Board certification or eligibility in Internal Medicine or Adult Primary Care Current New York State license or eligibility for licensure Experience in community health or outpatient primary care preferred Excellent communication and interpersonal skills Bilingual Creole/English strongly preferred #J-18808-Ljbffr Lasante-Health-Center
