Role Description The Community Liaison is a full-time hybrid role based in the San Antonio, Texas Metropolitan Area, with a mix of community-based work and some work-from-home flexibility. This role involves building and maintaining relationships with referral sources, community organizations, and healthcare providers to increase awareness of hospice and home care services. Day-to-day tasks include conducting outreach visits, participating in community events, providing education about hospice care, and responding promptly to inquiries from families and partners. The Community Liaison collaborates with clinical and administrative teams to coordinate patient referrals, ensure smooth transitions into care, and support a positive experience for patients and families. This position also requires maintaining accurate records of outreach activities, preparing reports, and using communication tools and technology to stay connected with internal and external stakeholders. Qualifications Strong community engagement and community outreach skills, with experience building partnerships and representing an organization in public settings. Excellent communication abilities, including clear verbal communication, active listening, and professional written correspondence. Proven customer service skills, with a focus on empathy, responsiveness, and resolving concerns in a supportive manner. Understanding of home care or hospice services and the ability to explain care options and processes to patients, families, and community partners. Ability to work effectively in a hybrid environment, managing time between field visits and remote tasks, and maintaining accurate documentation. Familiarity with the San Antonio, Texas community and local healthcare landscape is preferred. Proficiency with basic office software and CRM or outreach tracking tools. High school diploma or equivalent required; additional education in health care, social services, marketing, or a related field is beneficial.
