CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. ADMIT-TO-DISCHARGE PHYSICAL THERAPIST - PRN

$55/Hour | Outpatient + Home Care | Flexible Weekday Schedule

Community Physical Therapy (CPT) is looking for a PRN Physical Therapist to join our Admit-to-Discharge team.

If you're looking for flexibility, variety, and the opportunity to truly follow your patients from evaluation all the way through discharge, this could be a great fit.

What Makes This Opportunity Stand Out?

$55/hour PRN rate

Monday-Friday flexibility, with scheduling between 7:00 AM-6:00 PM

Outpatient + home care patients

Follow your patients from evaluation through discharge

What We're Looking For

Active Illinois Physical Therapy license

Outpatient or home care experience is a plus

Someone dependable, flexible, and comfortable working independently
