Occupational Therapist

Caledonia Home Health Care & Hospice is seeking an Occupational Therapist to provide in-home therapy services aimed at maximizing the independence and safety of our home care and hospice patients. The successful candidate must be able to work both independently and as part of a collaborative multidisciplinary team.

Schedule: Monday - Friday

Responsibilities:

• Provide comprehensive in-home therapy services to patients.
• Develop and implement individualized therapy plans.
• Assess patient needs and progress in rehabilitation.
• Collaborate with a multidisciplinary team to ensure comprehensive care.
• Maintain accurate and up-to-date patient records.

Required Experience / Certifications / Licensure:

• Valid Vermont licensure as an Occupational Therapist.
• A valid driver's license and reliable vehicle.
• Experience in a home care setting is preferred.

Assignment Duration: This is a 13-week contract position.
