In Home Healthcare LPN

Aveanna Healthcare seeks an In Home Healthcare LPN to deliver compassionate skilled nursing care to pediatric and adult patients in their homes. The LPN will perform assessments, administer medications and treatments, manage wounds and IVs, and monitor chronic conditions under RN or physician direction. Responsibilities include executing care plans, documenting in the EMR, educating patients and families, and collaborating with the care team. This role offers one-on-one patient care, clinical mentorship, ongoing training, and growth opportunities within a patient-focused, team-oriented national home health organization. Provide skilled nursing care to patients in their homes per care plan Perform assessments, monitor vital signs, and report changes in condition Administer medications, treatments, wound care, and IV therapies Document all care accurately and timely in the EMR system Educate patients and families on disease management and home care Collaborate with RNs, therapists, physicians, and care team members Follow infection control, safety, and regulatory standards Participate in ongoing training and competency development Home health nursing Medication administration Wound care IV therapy and injections Tracheostomy and ventilator care Patient and family education Care planning and documentation Vital signs monitoring Electronic health record (EHR) documentation Infection control Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN license in state of practice Current BLS/CPR certification Home health experience preferred Pediatric or adult skilled nursing experience a plus Valid driver's license and reliable transportation #J-18808-Ljbffr Nysurgeon
