A Center for Independent Living in Pennsylvania is seeking a Nursing Home Transition Coordinator to assist individuals with disabilities in transitioning back into the community. This is an hourly position requiring a BA in Human Services and strong organizational and communication skills. The ideal candidate will have a commitment to the independent living philosophy and must be able to work under pressure.
