RN - Case Manager

Case Manager (CM)

We are seeking a qualified Case Manager to join our team. The successful candidate will be responsible for managing patient care through all aspects, ensuring efficient and effective planning and delivery of healthcare services.

Key Responsibilities

• Develop and implement individualized care plans for patients.
• Collaborate with healthcare teams to coordinate patient care.
• Conduct assessments and evaluations of patient needs.
• Provide support and education to patients and their families.
• Monitor patient progress and adjust care plans as necessary.
• Document all patient interactions and care plans accurately in the electronic medical record system.
• Assist with the transition of patients between levels of care as needed.

Qualifications

• Active LSW (Licensed Social Worker) in the state of practice.
• Preferred experience in Acute Rehabilitation Unit (ARU) settings; hospital experience may be considered.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficient in utilizing electronic medical records software.

Work Schedule

The position will operate on a day shift schedule from 08:00 AM to 04:30 PM.

Additional Information

• Position requires flexibility to float to other units if necessary, with opportunities available to float to Long Term Acute Care (LTACH) as openings arise.
• The facility accommodates 11 beds in total.
• Scrub colors are navy blue.
• Parking is available in the main lot.
