The Intake Liaison plays a crucial role in ensuring the smooth operation of our business development firm by managing patient intake processes. This position requires strong organizational skills, attention to detail, and a compassionate approach to patient care. The Intake Liaison will be responsible for collaborating with home care agencies and their respective intake team, assisting patients with scheduling NYIA appointments, managing case load, and facilitating communication between patients and healthcare providers.

Duties

• Manage the patient intake process, including scheduling NYIA appointments and verifying insurance information.
• Assist caregivers with their registration process.
• Communicate effectively with patients regarding their care plans and any necessary documentation.
• Update the account management and marketing team on the progress of their case load.
• Operate phone systems to handle incoming calls, answer inquiries, and direct calls as needed.
• Collaborate with the home care agency's intake, coordination, and HR staff to ensure seamless patient transitions and continuity of care.
• Provide support in office management tasks as required.

Requirements

• Experience in home care intake experience is preferred.
• Familiarity with eMedNY Epaces is a plus.
• Knowledge of Health insurance and long-term care eligibility is advantageous.
• Strong communication skills, both verbal and written, to interact effectively with patients and staff.
• Ability to handle sensitive information with confidentiality and professionalism.
• Proficient in using phone systems for effective communication within a busy medical office environment.
• A compassionate demeanor with a focus on providing excellent patient care.

This role is essential for maintaining the high standards of our customer service while ensuring that patients receive the support, they need from their first point of contact.

Job Type: Full-time

Salary Range: $48,000- $53,000
