SCHEDULE - Two days a week from 7:30AM-6PM - WEDNESDAY & EITHER TUESDAY OR THURSDAY.

New Journeys (NJ) is a voluntary, intensive early intervention program that provides coordinated specialty care for individuals experiencing their first episode of psychosis. A small interdisciplinary program works with individuals and their natural supports to assist participants in achieving optimal functioning in home, school, work, social and leisure areas of their life. The NJ Team provides individual resilience training/individual training (IRT), education and employment support, family support and education, peer support, case management, and medication management. Treatment can involve multiple contacts per week with the individual and/or natural supports, and can occur in the office, community, or in the individual's home. Enrollment in New Journeys can last up to two years with the goal that individuals are in the recovery phase of psychosis and confident in their abilities to live a fulfilling, meaningful life upon graduation.

The New Journeys nurse supports healthy behaviors, wellness strategies, and care coordination. The nurse will conduct comprehensive nursing assessments, identify chronic disease risk factors, and provide education and self-management support focused on regular physical activity, healthy nutrition, sleep hygiene, and overall wellness. The New Journeys nurse ensures that recommended metabolic monitoring is completed in a timely manner and tracked in FBH and WSU measurement tools. The Nurse has a critical role in supporting medication management, provides education on prescribed medications, monitoring for side effects and implementing strategies to promote medication adherence. Through ongoing assessment, education, and collaboration with the treatment team, the nurse supports individuals in achieving optimal health outcomes and improving their overall quality of life.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following :

• Meet with clients weekly for the first month, every 2 weeks for the next 5 months and monthly for the remainder of client's enrollment with New Journeys.
• Provide symptom and medication assessment, evaluation, education, and management to individual with persistent mental illness. This includes consultation with Psychiatrist, MHCP, hospital, community providers, client, and client's family/guardian for care.
• Provide health education and coaching to clients to support healthy lifestyle and client activation.
• Process and sign medication orders, complete medication refills and prior authorization for specific medications based on provider request.
• Administer medications to consumers as directed by FBH Psychiatrist.
• Ensure access to medications through timely refills, by obtaining sample medication per order and by appropriate use of FBH discretionary funds or patient assistance programs for medications.
• Ensure pertinent medication related information is conveyed to hospital staff in the event of client hospitalization.
• Assist in development and delivery of staff and/or consumer education in the form of group classes or trainings.
• Identify and include the client's support network and applicable involved community partners in care coordination.
• Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
• Assist clients in the development of healthy coping mechanisms in individual or group settings.
• Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
• Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
• Represent the agency and its services to the community in a positive and professional manner.
• Participate in any required training, consultation calls, and onsite visits or review when required by contract.
• Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring or training responsibilities.
• Establish and maintain collaborative working relationships with; clinical, administrative, support staff, community partners, clients, and family/natural supports.
• Demonstrate teamwork and goal-oriented productivity.
• Perform FBH charting requirements, correspondence, Workday and other paperwork in a complete and timely manner.
• Operate as a member of multi-disciplinary team and ensure services are provided in the community and clinical setting as required by contract.
• Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle.
• Other duties as assigned.

QUALIFIED APPLICANTS WILL HAVE:

REQUIRED:

• RN licensed in the State of Washington
• Active BLS within 90 days of hire and must maintain active BLS
• Must have an unrestricted and valid driver's license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.

PREFERRED:

• One year of experience working with individuals with severe mental illness.

KNOWLEDGE, SKILLS & ABILITIES:

• Knowledge of Washington Administrative Codes and statues pertaining to the treatment and delivery of mental health services, or ability to obtain within 3 months of employment.
• Knowledge of service delivery models including groups, skills training brief/focused therapy, community-based interventions for individuals with serious and persistent mental illness, or ability to obtain within 3 months of employment.
• Ability to communicate and engage effectively with individuals of varied backgrounds, trauma experiences, cultures, education level, and socio-economic status.
• Must possess excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.
• Provide health education using client centered and trauma informed practices.

PHYSICAL, SENSORY & ENVIRONMENT:

• This position provides client services in community settings on a daily basis.
• Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.
• Some environments may not be ideal in terms of cleanliness or being habitable.
• Daily travel between locations is required.
• The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
• Daily use of a computer is required.
• May be required to lift 10 - 20 pounds.
• Blood-borne Pathogen Category IV - No exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt): $39.61- $48.26

• All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $39.61 per hour.
• .5 FTE = ?20 hours per week
