Together, we’re building a stronger, more connected healthcare system. About the Role Location: Riverside (must be able to work on-site). This position is a Nurse Practitioner (NP) responsible for comprehensive health assessments. Key duties include: Obtain patient history and perform a physical examination. Complete review of medicines and related instructions. Review home safety and psychosocial needs. Fulfill documentation needs for STAR and HCC measures. Obtain and document all pertinent information in patient charts and case management notes on the same day as patient is seen; include medications, end-of-life preferences, and “drivers” of preventable hospital admissions. Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies. Maintain patient safety and confidentiality; use universal precautions in all patient care. Prescribe/order procedures, treatments and medications within scope of practice. Develop a high level of trust and intimacy with patients and families. The Skills, Experience & Education You Bring • CA Licensed Nurse Practitioner • 1+ Years of Experience with Primary Care The Base Salary for this Position $130,000 - $150,000 per year.
