Occupational Therapist OT

Occupational Therapist (OT) Job Description: The Occupational Therapist plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents. Responsibilities: Assessment and Evaluation: Conduct comprehensive physical assessments todetermineresidents' physical impairments and functional limitations, focusing on mobility, balance, and gait. Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents'functionalmobility. Treatment: Administer therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function. Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.Conducts remote check-in through remote therapeuticmonitoring(RTM) with discharged patients to home or next level of care. Documentation:Comfortable and knowledgeable with Point ofService(PoS) care delivery and documentation.Maintain detailed and up-to-date documentation of resident evaluations, treatment plans,dailyand progress notes, HMO/Manage care progress updatesinEMRs(InnovaHealth/Net Health/PCC). Interdisciplinary Collaboration: Collaborate with the rehab team, including Physical Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents. Qualifications: State licensure asanOccupational Therapist. EMR and MS office applications. CPR/BLS certification. Knowledge of Section GG data collection and documentation requirements. Strong clinical and interpersonal skills.Excellent written and verbal communication skills. Ability to supervise assistants,technicians/aidsand student/interns. Modality Certification + Experience in LTPAC/LTC clinical programs are always a plus
