Medication Reconciliation Assistant
The Medication Reconciliation Assistant (MRA) practices within the health system and works under the supervision of a pharmacist to collect and document the best possible medication list, including the correct drug name, strength, route, and administration frequency for all current prescription and over-the-counter medications.
The MRA will interview patients, their families, external medical providers/facilities, and/or outpatient pharmacies to confirm the accuracy of the medication list.
The MRA coordinates corrections to a patient's medication list by notifying physicians and/or pharmacists of updated information.
The MRA may obtain, add, or correct medication allergy information including the medication name and type of reaction.
Under the general supervision of a pharmacist, the Medication Reconciliation Assistant is responsible for the collection of the best possible medication list.
The best possible medication list will include the drug name, strength, route, and frequency of administration for all prescription and over-the-counter medications.
The process of collecting this information may include interviewing patients, their family members/caregivers, external medical providers/facilities, and outpatient/mail-order pharmacies.
Additionally, a review of electronic medical records from prior hospital encounters may be needed to collect and/or validate information.
Upon obtaining the medication list, the MRA is responsible for an accurate documentation of all information in the electronic medical record for the current hospital encounter.
All corrections or additions to any previously collected medication list will be communicated to the medical provider for appropriate review and action.
Responds timely to medical providers and patients needs by answering phone calls, receiving and distributing messages, and triaging requests appropriately.
Understands and implements appropriate time management and prioritization of tasks ensuring timely and safe patient care.
Participates in quality improvements projects to enhance the techniques and processes used to capture the best possible medication list.
Collects, analyzes, and presents data to internal and external committees/associations relative to work processes and procedures associated with quality improvement projects.
Creates, revises, and/or presents educational materials to providers, nurses, ancillary staff, and new employees to improve patient safety and quality of care.
Trains new employees on medication history standard workflow.
Participates in committees and initiatives relative to the advancement of the Medication Reconciliation Program.
Serves as a liaison between the Medication Reconciliation Team and the hospital clinical staff to enhance interdisciplinary communication surrounding the processes involved in gathering the best possible medication history.
Provides support to less experienced technicians and assists in the training of new personnel.
Orients and trains new personnel.
Acts as a resource for less experienced staff, providing guidance for medication history collection.
May complete and sign off on Orientation Documentation Check Lists.
Follows and implements Emory Healthcare PLEDGE and care transformational model in all activities and interactions.

Attends educational in-services as appropriate.
Other duties as assigned.
Minimum Qualifications:
- Two years of college.
- Successful completion of an ASHP-accredited technician training program and current certification by passing the CPhT national exam or currently enrolled in accredited School of Pharmacy with current Georgia Pharmacy Intern license is required.
- Must have 2 years of experience as a Pharmacy Technician.
- Combination of education and/or experience may be considered in lieu of college requirement.
- Current Registration with the Georgia Board of Pharmacy.
- Must maintain certification and registration through entire length of employment.
Physical Requirements (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks. Environmental factors: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
