Local Travel Home Health Physical Therapist

The company is hiring an experienced local travel home health physical therapist for a 13-week contract in Springfield, Missouri. The role offers competitive weekly compensation up to $2,700 and flexible travel opportunities while allowing the PT to make a direct impact on patients in the home health setting.

Key Responsibilities:

• Conduct comprehensive patient evaluations in the home health setting.
• Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
• Perform start-of-care assessments, recertifications, and routine home health visits.
• Complete accurate OASIS documentation and maintain compliance with home health regulations.
• Serve as a case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams.
• Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions.
• Monitor patient progress and modify treatment plans as needed.
• Accurately document all patient interactions and treatment outcomes using Matrix EMR.
• Deliver compassionate, patient-centered care while promoting functional independence and quality of life.

Qualifications:

• Doctorate or Master's degree in Physical Therapy from an accredited program
• Active Missouri Physical Therapist license required
• Strong experience with OASIS documentation, case management, start-of-care assessments, and recertifications required
• Excellent communication, organizational, and time-management skills
• Ability to work independently in a home health environment

Preferred Skills:

• Current CPR/BLS certification preferred
• Home health experience highly preferred for travel assignments
• Experience with Matrix EMR preferred
