RN I: Care Management & Transitions Specialist

Horizon Blue Cross Blue Shield of New Jersey is seeking an RN responsible for care management and ensuring high quality medical services for patients. You will evaluate patient needs and collaborate with a multidisciplinary team to coordinate care effectively. The ideal candidate will have an associate’s or bachelor’s degree in nursing along with at least two years of clinical experience, plus an active New Jersey Registered Nurse License.
