Job Summary
The Oncology Nurse Navigator is a licensed nurse specializing in cancer clinical care coordination and care management, based in Palo Alto. This position involves providing nurse-led navigation throughout the patient journey from diagnosis to survivorship, collaborating closely with the clinical team to optimize care and improve patient outcomes. The role emphasizes holistic, culturally sensitive support and effective communication within the interdisciplinary team.

Job Details

• Location: Palo Alto, California
• Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week
• Required Qualifications: Bachelor's Degree of Science in Nursing (BSN) from an accredited college/university; CA-RN license; minimum of 5 years of nursing experience; at least 2-3 years in oncology; experience with nurse navigation and ambulatory oncology case management; strong clinical knowledge of cancer disease processes, treatment, symptom management, and supportive care; proficient in medical terminology; strong problem-solving, decision-making, and critical-thinking skills; experience with MS Office.
• Preferred Qualifications: MS or MSN degree; Oncology Nurse Certification (OCN) preferred; Advanced Oncology Certified Clinical Nurse Specialist (AOCNS) preferred; experience with Epic EMR; ability to develop educational materials involving graphics and tables; excellent interpersonal and organizational skills.
• Additional Information: The role involves assessment of patient education needs, addressing clinical and systemic barriers, facilitating communication among the care team, providing culturally appropriate education, screening for distress, and ensuring quality standards meet accreditation requirements. The position requires collaboration with social work and nutrition teams, and serving as a liaison to relevant professional organizations.
