Physical Therapist Outpatient

Tryfacta is seeking a Physical Therapist Outpatient for our client in Monticello, IN 47960. This is a 13-week contract assignment with high possibility of extension.

Position Title: Physical Therapist Outpatient

Location: Monticello, IN 47960

Duration: 13 weeks

Work Schedule: Day 5x8-Hour (08:00 - 16:30)

Pay rate: $58 to $62 per hour

Responsibilities for this position include, but are not limited to:

• Evaluate patients and develop individualized treatment plans and goals.
• Provide physical therapy for orthopedic, pediatric, acute care, and outpatient conditions.
• Perform gait training, therapeutic exercises, mobilization, traction, taping, and post-operative rehabilitation.
• Assist patients with mobility, transfers, balance, strength, and functional independence.
• Recommend and train patients in the use of assistive devices and adaptive equipment.
• Monitor progress, conduct reassessments, and modify treatment plans as needed.
• Maintain accurate documentation, including evaluations, progress notes, goals, plans of care, and discharge summaries.
• Educate patients and caregivers on exercises, safety, and home programs.

• Is on-call REQ? Yes, occasional Saturday coverage.
• Preferred, Cerner
• Certification required: BLS, Manual Therapy
• 1+Year Experience Required
