Job Summary

The Community Resource Center (CRC) Community Health Coordinator (CHC) II is responsible for promoting member's optimal well-being through active engagement and assisting in the navigation of their access to health services at one or more assigned CRCs. The CHC supports the CRC team through an integrated approach to the CRC visitor navigation, engagement and community outreach utilizing health screening assessment tools and orientations presentations. This position focuses on L.A. Care's members and includes daily interactions with non-member visitors' to the CRCs. The CHC may provide general health education/ information and support the center by identifying the visitors' or members' barriers to care and their preferences, mentoring and connects them to resources and act as an advocate for the individual.

Duties

• Cultural Mediation Among Individuals, Communities, and Health and Social Service Systems: Orients and explains to members and visitors on how to use health and social service systems which includes how the system operates. Responsible for the new health plan member orientation and the new CRC new visitor orientation. Assists in building health literacy and cross-cultural communication.
• Providing Culturally Appropriate Health Education and Information: Assists in conducting health promotion and chronic disease prevention education in a manner that supports evidence based curricula offered at CRCs and matches linguistic and cultural needs of members. Provides the members/ visitors general health information to facilitate understanding on disease prevention and how to manage health conditions that includes chronic disease.
• System Navigation: Makes referrals to onsite or community resources and provides follow-up. When necessary, contacts are transferred to CRC or Customer Service Center team staff onsite for more advanced support. Coordinates member transportation to services and help address barriers to services. Informs people and systems about community assets and challenges. Includes providing support to members with completion and submission of forms required to identify a designated legal representative for health care decisions.
• Providing Coaching and Social Support: Provides support and coaching to members/visitors through motivational interviewing. Motivates and encourages the members/ visitors to obtain care and other health and social services. Responsible for the administration of social determinants of health assessments for health plan member or CRC visitors. Supports self-management of chronic disease prevention and management of health conditions.
• Advocating for Individuals and Communities: Advocates for the needs and perspectives of members/ visitors and communities by assisting them to connect to resources and advocates for basic needs (e.g. food and housing).
• Building Individual and Community Capacity: Builds individual and community capacity through training and building individual capacity with peers and CHW groups.
• Implementing Individual and Community Assessments: Participates in the design, implementation, and interpretation of individual and community-level assessments, social determinants of health assessments. (e.g. windshield survey of community assets and challenges, community asset mapping)
• Conducting Outreach: Case-finding/recruitment of individuals, families, and community groups to services and systems. Follows up on health and social service encounters with individuals, families, and community groups. Presents at local agencies and community events.
• Attends regular staff meetings, on-site monthly trainings and other meetings as requested.
• Performs other duties as assigned.

High School Diploma/or High School Equivalency Certificate

Experience Required

At least 1 year of experience as a community health worker, health navigator, peer support worker, outreach worker, or promotora or working in a health care, social work or community-based setting.

Skills Required

Knowledge of community resources in area of residence. Comfortable working with diverse populations. Exceptional ability to connect and engage with people. Able to work flexible job hours. Excellent verbal and written communication skills. Detail oriented, organized and possess time management skills. Basic computer skills.

Required Training

Training in health education for chronic diseases. Successful completion of a Community Health Worker certification program from a college or other educational institution.
