On-Site RPM Care Manager — Four-Day Week
EPIC Health System LLC
EPIC Health is seeking a dedicated RPM Care Manager to monitor patient data and engage patients in their care within an on-site Southfield, MI office. You will coordinate timely clinical follow-up for patients with chronic conditions and support education and device usage for RPM participants. The role requires a high school diploma or GED, healthcare experience, and strong communication skills.
