Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Identify and reassess cost-efficient levels of care
• Coordinate non-medical support services such as housing and transportation
• Facilitate care transitions in coordination with healthcare professionals
• Engage specialty resources for optimal patient outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Promote health and prevent illness through targeted initiatives
• Act as patient advocate, safeguarding privacy and confidentiality

Requirements of the RN - Case Management

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Ability to analyze complex medical information and make informed decisions
