Key Responsibilities This team conducts post-discharge outreach to AZCH Members post hospital or facility release. Goal to ensure services/appointments are in place to prevent readmission. Team culture is positive, collaborative, and supportive. Key projects include helping to improve Plan All-Cause Readmission Rates and Follow Up After Hospitalization Rates. This is a temp role to support the team while key leader is out of office on LOA. In the morning, staff receive list of members needing post-hospital call. The staff is required to make 1st call and set out reminder task for 2nd call if unable to contact. If successful contact is made, staff will complete post hospital assessment, help close any identified gaps, make referral to CM if appropriate, provide education about self-management resources, and complete appropriate documentation. Any follow up calls for unable to reach members will be completed and mailing of UTR letter if needed. Staff are assigned daily calls however also have follow up call. We are reassessing the volume of calls in comparison to overall reach rate and other team priorities/needs. This target may increase/decrease depending on identified efficiencies. This role is unique in that it carries no caseload and is a critical part of daily health plan operations. Key Requirements and Technology Experience Key Skills: 2-4 years of related experience, in a medical or behavioral health office setting, with member/provider-facing responsibilities. Case Manager, Behavioral Health Case Manager, Medical Assistant, experience with Medicaid/Medicare members; health insurance experience is a plus. Health-related experience (Medical or Behavioral Health) and direct patient or client contact experience is required. Team player with strong communication skills and willingness to learn.
