Behavioral health experience does not apply. Description: Position for a registered nurse specializing in case management. The role involves managing patients' care coordination and discharge planning. Float expectations are not required for this position. A seasoned traveler is preferred, as first-time travelers are not accepted. The patient ratio during daytime shifts is 1:25 and 1:45 on weekends. Experience with inpatient care is essential. Prior experience needs to include assessing patients and participation in family meetings. The candidate will be involved in developing treatment plans and coordination with interdisciplinary teams. Knowledge of HMOs, IPAs, and community resources is beneficial. Candidates should have experience with advanced directives, protective services processes, and arranging durable medical equipment. The responsibilities include managing resources effectively for diverse age groups, including neonates, adolescents, adults, and geriatrics. The daily census is expected to be around 25 patients. Strong organizational skills and the ability to work independently are key for this role. The candidate should be competent in using computerized documentation systems. Must be able to perform all tasks without direct supervision. Willingness to meet departmental needs requires weekend duty every other weekend. Commitment to working three out of five major holidays is mandatory. Compliance with all requested time-off schedules during submission is necessary. Refusal to float may lead to termination of the assignment.
