Job Description
PURPOSE OF THE INTELLECTUAL DISABILITY DEPARTMENT:
To provide services to promote and support independence in the lives of individuals who have an intellectual disability. To enable them to live full quality lives as an integrated member of their community.
DUTIES AND RESPONSIBILITIES:
The Certified Nursing Assistant or Certified Medical Assistant is a key individual responsible for monitoring and reporting changes in the individuals’ condition to the LPN, assisting the LPN in maintaining accurate patient records, monitoring vital signs, helping the LPN with tasks, and offer support for people so they can live where they wish in the community, live a quality life and be as independent as possible.
The CNA or CMA is responsible for assisting the LPN in ensuring the health and wellbeing of the individuals that Bell Socialization Services serve within the Residential Department. This includes but is not limited to helping the LPN review medical paperwork, monitoring all individuals at least monthly, and attending appointments and meetings as needed. In addition, the CNA or CMA, will assist the LPN in conjunction with other management staff, ensuring that all Residential Group homes maintain consistent compliance with all applicable state regulations set forth by the Office of Developmental Programs.
Because of the nature of this Agency, it is essential that the CNA or CMA believe in the philosophy of the Agency as well as support the procedures that are viable and essential to the individuals’ health needs.
CRITICAL ELEMENTS:
1) Ensure the Safety of Staff and Individuals. The CNA or CMA must: promptly report all safety issues and concerns to the LPN and/or appropriate office staff; monitor the sites to ensure the programs are operating in a safe manner; and provide support for the RPS’s and DSP’s to maximize safety. Examples of duties in this area:
a) Ensuring the health, safety, and wellbeing of the individuals served
b) Report to the Director of Services, Assistant Director of Services, and LPN
c) Assess the individual(s) safety needs and provide services and supports that will maximize their safety.
d) Administer insulin as needed when staff who are trained through WellSpan Diabetes Center are not available
e) Assist the LPN train staff to administer non-oral medications to ensure proper administration techniques are being utilized in coordination with the Training Coordinator.
f) Check monthly medication cycle refills along with other office staff to ensure accuracy and that all medication is delivered as specified.
g) Ensure that the Medication Administration Record is accurate and matches the labels on medications sent for the month
h) Assist the LPN to ensure all staff that work with an individual are trained in any new diagnoses or medications (side effects) that individual receives
i) Become a Certified Investigator under ODP guidelines as classes become available and maintain certification
2) Ensure the Individual’s Needs are Met: The CNA or CMA will get to know each individual to identify their needs and determine what type of assistance is necessary to support the individual in their home. The CNA or CMA will work with the LPN, Training Coordinator, and Program Coordinators to ensure that all individuals’ health needs are being met. Examples of duties in this area:
a) Monitor each individual face to face on a monthly basis- blood pressure, pulse, foot checks, and weight
b) Monitor all adaptive equipment on a monthly basis to ensure good repair and working condition
c) Accurately keep track of inventory including medical supplies and equipment to prevent shortages and delays in care helping to promote safety of staff and individuals.
d) Attend Medical appointments with the individual and staff as needed
e) Encourage individuals to express their medical concerns and needs
3) Communicate with Others: The CNA or CMA must represent Bell in a professional manner. The CNA or CMA must act as an integral member of the Management Team and must keep individuals, staff, case management, families, Director of Services and other identified stakeholders informed of health issues. Additionally, the CNA or CMA must process written communication for various internal departments in a timely and accurate fashion as determined by the department. Examples of duties in this area:
a) Interact professionally and positively with others- this includes the individual, family, personnel from other agencies, medical personnel, staff in the homes, and coworkers
b) Assist the LPN with Informing staff, supervisors, and management of significant information
c) Ensure that any incidents reported to the CNA or CMA are reported to the Incident Manager in accordance with state regulations.
d) Assist the LPN with providing staff members the necessary training and information needed to successfully carry out their duties.
e) Carry out the directives of the Director of Services, Assistance Director of Services, and LPN
f) Assist the LPN with keeping medical documentation filed and up to date
4) Ensure the Program Sites are in Compliance with All Regulations: The CNA or CMA must be knowledgeable and informed on all applicable State Regulations, Bulletins, and Health Alerts. The CNA or CMA must monitor multiple individuals with regards to compliance with these regulations. Examples of duties in this area are:
a) Assist the LPN with reviewing monthly medical appointments for accuracy in documentation of all appointments and follow up visits
b) Assist the LPN with reviewing monthly medication reviews and MAR’s for accuracy in documentation of medications, for appropriate documentation of medication errors, and for all necessary signatures
c) Maintain a current Pennsylvania Certification as a CNA or CMA
d) Assist the LPN with reviewing all monthly medical paperwork to ensure that all forms are complete and correct and that all needs documented have been fulfilled
5) Ensure Programs Offer Services in a Manner Consistent with Bell’s Philosophies: The CNA or CMA must be knowledgeable with Bell’s Policy and Procedures Manual as well as philosophical beliefs. The LPN will supervise the CNA or CMA in monitoring program sites to ensure services provided are consistent with policies, procedures and philosophies of the Agency. In the Intellectual Disability Department, philosophies utilized include individual directed services, person centered planning, positive approaches, normalization, and everyday lives philosophies and behavioral psychology. Examples of duties in this area:
a) Refrain from wearing a uniform
b) Encourage independence for individuals with medications
c) Involve individuals in their medical needs
d) Attend training to develop skills and knowledge.
e) Assist the LPN with coordinating training for staff personnel as needed
f) Encourage involvement of individuals’ families.
g) Support individual’s choices.
ADDITIONAL ELEMENTS:
1) Participate in department meetings.
2) Support other programs in the Intellectual Disability Department.
3) Function as part of Bell’s management team.
4) Function as part of a team within the Residential ID Department
5) Gain 24 hours of training each year
6) Change schedule as needed to meet individual needs
7) Other tasks as assigned by LPN, ADOS, or DOS
8) Attend meetings as requested
Requirements:Minimum Qualifications: HS diploma/GED, current state-issued certificate and two (2) years professional experience; proficient understanding of medical terminology; strong organizational skills; attention to detail; ability to complete physical and PPD tests. Must be at least 21 years old, possess valid driver’s license, insurable driving record, and have reliable transportation. Current physical and Mantoux (TB) test required. PA State Police background check, Act 33 and/or FBI clearances. E.O.E.
