Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand each client's unique circumstances and develop effective treatment plans.

• Maintain accurate and timely documentation of clinical notes, patient interactions, and progress in accordance with departmental policies and electronic health record standards.
\n
• Collaborate closely with referring physicians and other healthcare providers to discuss patient progress, address any concerns, and adjust treatment plans as necessary.

• Offer crisis intervention services and therapeutic support for clients experiencing emotional or behavioral challenges, guiding them to appropriate resources and coping strategies.
\n
• Facilitate care coordination by working with clients, their families, and community resources to develop comprehensive discharge plans and ensure continuity of care.

• Engage in regular consultation with clients' support systems to foster an understanding of treatment goals and collaborative planning strategies.

Patient Population / Client Focus\n

In this role, you will primarily serve adult clients with a variety of mental health concerns including anxiety, depression, trauma, and adjustment disorders. Our patient population is diverse, representing various cultural, social, and economic backgrounds. Qualifications\n

• A Master’s degree in Social Work (MSW) is required, ensuring a strong foundation in clinical practices.

• A California state license as a Licensed Clinical Social Worker (LCSW) is mandatory to deliver clinical services.

• Minimum of two years of experience in an outpatient mental health setting, demonstrating proficiency in therapy and clinical assessments.

• Strong understanding of electronic medical record systems, with demonstrable skills in managing patient documentation efficiently.

• Excellent interpersonal skills and the ability to communicate effectively with clients, families, and interdisciplinary teams.

• Self-motivated with strong organizational abilities, enabling independent work while staying accountable to clients’ needs.

Preferred Experience\n

• Experience in providing teletherapy or using telehealth platforms is preferred, as is familiarity with relevant technologies for remote service delivery.

• Background in crisis intervention and knowledge of community resources to address patient needs effectively.

• Proficiency in various therapeutic modalities, including CBT and DBT, will be advantageous.

Compensation and Benefits\n
