It is committed to equity and inclusion for all employees. Position Summary Advanced Practice Provider Nurse Practitioner Hemonc - Huntington. The Nurse Practitioner will plan and deliver safe, therapeutic, population-specific patient care, working collaboratively with an interdisciplinary health team to optimize outcomes and promote wellness. Job Responsibilities Perform other duties as assigned. Plan and organize care to meet individual patient needs and ensure proper resource utilization. Collaborate with the attending physician to develop and implement the therapeutic plan for a group of patients and ensure communication with family members. Participate in the development and revision of interdisciplinary and nursing standards and educational materials as relevant. Maintain timely and appropriate communication with family members, house staff, other disciplines, and ancillary services. Communicate with nursing leadership regarding the nursing staff. Provide consultation to nursing staff regarding therapeutic regimens. Facilitate continuity of care across services, including home care and community follow-up. Demonstrate knowledge of the organization’s Service Standards and incorporate them into practice. Participate in professional nursing standards and regulations related outside agencies. Act as a respectful member of the healthcare team. Conduct a complete health history, physical examination, and psychosocial assessment. Order laboratory, radiological, and diagnostic studies per protocol and interpret results, including referrals when necessary. Formulate patient plans of care with attending physician and family based on goals and length of stay. Collaborate with physicians and other disciplines to maximize patient/family preferences and improve quality, access, and cost-effectiveness. Prescribe in-patient and discharge medications, including controlled substances with DEA authorization. Prescribe medical equipment, devices, therapy, and home health services per protocol. Practice within the medical staff bylaws and privilege delineations. Evaluate treatment effectiveness and modify plans per standards and protocols. Consider cultural and age-specific needs in care. Promote professional growth and development. Maintain current expertise in area of practice. Serve as a clinical expert and resource for peer education. Monitor and track cost-effectiveness of the position relative to performance indicators. Manage Ostomy patients in the outpatient setting. Minimum Qualifications Master’s Degree graduate of an accredited Nurse Practitioner program. Current NYS Registered Nurse License. Current NYS Nurse Practitioner License. Current Nurse Practitioner Certification. Excellent communication, interpersonal, and organizational skills. Basic computer proficiency. Basic Life Support Certification through the American Heart Association. Preferred Qualifications Advanced Life Support certification (ACLS, PALS) through the American Heart Association. Neonatal Resuscitation certification (NRP) through the American Academy of Pediatrics. Other Requirements Qualified candidates must effectively communicate with all levels of the organization.
