Responsibilities Assesses, plans, implements, evaluates, and supervises individual patient care on a nursing unit/department according to unit policies and procedures. Provides supervision of non-RN personnel on the assigned unit. When assuming the charge role, delegates and supervises unit activities. May be required to work on other nursing units according to staff and patient distribution. Performs other duties as assigned or requested to maintain a high level of service. Assesses the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care. Develops, communicates and documents a plan of care reflecting individualized patient problems and interventions to achieve desired outcomes, involving the patient, family and healthcare team. Implements and coordinates the plan of care in collaboration with the healthcare team. Evaluates the effectiveness of the plan of care, patient response to care, and readiness for discharge. Revises the plan of care as needed and makes referrals where indicated. Plans for education after assessing the individual learning needs and readiness of the patient and family. Provides educational opportunities and documents outcomes. Functions as a preceptor to new staff. Functions as a charge nurse. Meets performance expectations for customer service, teamwork, resource utilization, and staff and self-development as outlined in performance review. Performs other duties as assigned or directed to ensure smooth operation of the department/unit. Demonstrates competency in the care of the neuro patient (assessment, airway management, trachea care, assistance during special neuro procedures, and use of equipment such as a halo brace and ventilator). Qualifications Education: graduation from an accredited school of nursing; BSN preferred. Current BCLS certification or obtain within one month of hire. Registered Nurse license in the State of Connecticut. Clinical specialty certification highly encouraged. Additional Information – Units Heart and Vascular Cardiac ICU (5-1 SP) Fast‑paced, high‑acuity 14‑bed critical care unit caring for adult patients with complex cardiac and medical diagnoses such as myocardial infarction, cardiogenic shock, unstable arrhythmias, and heart failure. Nursing staff trained in advanced heart failure management and mechanical circulatory support devices. RN-HVC Cardiothoracic ICU (CTICU/WP3) 18‑bed cardiac surgery unit specializing in post‑operative CABG, valve replacements, aortic dissection repairs, heart transplants, and LVAD implantation. Only unit in YNHH health system caring for patients requiring ECMO, IABP, Impella/VADs, and/or CRRT. HVC Cardiovascular ICU and Stepdown (5-4 SP) 14‑bed hybrid unit with 8 ICU and 6 stepdown beds, caring for post‑complex vascular surgery and post‑cardiac surgery patients such as CABG and valve replacements. #J-18808-Ljbffr
