LPN - Home Health

Schedule

Assignment Details

Contract length: 13 weeks; Guaranteed hours: 40 per week

Requirements

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN in a clinical setting, preferably in home health or hospice
• Proof of current Hepatitis consent/declination and TB vaccination record
• Ability to use electronic medical charting and time-keeping software
• Reliable transportation and current auto liability insurance coverage
• Fluent in reading, writing, and speaking English

Responsibilities

• Provide skilled nursing care to patients at their residence, as prescribed by a physician.
• Assist in identifying patients' physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing patient care in collaboration with the RN.
• Ensure effective communication with patients, families, and interdisciplinary team members.
• Teach patients and families appropriate self-care techniques and assess their response.
• Follow infection control measures to protect the safety of patients and staff.
• Assist with end-of-life care for hospice patients as needed.
• Assist with the admission, discharge, and transfer processes for patients.
• Maintain professional boundaries in relationships with patients, families, and colleagues.
• Participate in annual in-service and performance improvement programs.
