Collaborate with multidisciplinary teams to ensure optimal patient outcomes.Location: Hoffman Estates, ILFacility: Ascension Alexian BrothersDepartment: Case ManagementSchedule: Day shift | Full-timeHow you'll make an impact in this role:Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient and family preferences and values.Monitor patient care through assessments, evaluations, and record reviews, serving as a resource for proper medical documentation, service appropriateness, and post-discharge treatment options.Advocate for necessary resources, eliminate care barriers, and maintain ongoing dialogue with supervisors and healthcare providers for effective plan implementation.Oversee patient progress and care quality while facilitating clear communication among all members of the healthcare team.What minimum requirements you'll need:Licensure / Certification / Registration:Required Credential(s): Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.Preferred Credential(s): BLS Provider.
