Licensed Practical Nurse (LPN) Job Type: Full Time | Shift: Days | 36 hour weeks (12 hour shift 7 – 7:30). How you'll contribute A Licensed Practical Nurse provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice. Essential Functions Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient. Performs patient care considering needs specific to the standard of care for patient age; includes geriatric and general inpatient rehabilitation patients. Under the supervision of the RN, assists with patient assessments and reassessments as per policy, identifies and initiates appropriate rehabilitation nursing interventions, and performs timely and accurate QI assessments. Carries out the plan of care as indicated by patient needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient condition and individual needs. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Demonstrates ability to assess and reassess pain, utilizes appropriate pain management techniques, and educates the patient and family regarding pain management. Other duties as assigned. Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program. Current state licensure as Licensed Practical Nurse.
