Clinical Supervisor: Physical Therapy Home Health

Coverage area: Greater Sacramento Please note: All opportunities at 21HHS require field visits to patients in their homes. Remote/work‑from‑home positions are not available unless specifically noted. Responsibilities Provide in‑field oversight of clinicians who deliver care and treatment to patients in their homes. Manage a hybrid schedule: 2 days in the Roseville or Natomas office (TBD) and 3 days in the field, visiting 12 patients per week for supervisory visits of field clinicians. Serve as the first point of contact for field clinicians, ensuring timely support and escalation of patient care issues. Handle incoming calls promptly and follow up to meet internal and external customer needs. Coordinate training and educational programs for nurses in the field to improve clinical outcomes and competencies. Assess staff learning needs, develop specific plans and goals for the next evaluation period, and serve as a role model and resource for staff. Coordinate bi‑weekly case conferences with the clinical team to discuss patient progress and revise care plans as needed. Review and evaluate each case by assessing services provided, reviewing records, and guiding clinicians to improve performance and quality of care. Review patients’ clinical diagnosis, medications, procedures, and clinical course. Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care. Qualifications At least two years of management or supervisory experience in a rehabilitation therapy department or program, preferably home care. Degree from a baccalaureate or master’s program in an applicable discipline approved by an accredited organization. Current California Physical Therapist license. Current Basic Life Support (BLS) certification. Valid California driver’s license with an acceptable driving record. All licenses and certifications must be current at the time of hire and sustained throughout employment. Benefits Wage range: $143,000 – $163,000 plus a generous bonus, paid quarterly.
