CNA - LTC/SNF

Job Summary:


Assist patients in long-term care or skilled nursing facility settings by providing essential clinical and supportive care, ensuring comfort and safety throughout their stay.

Responsibilities:

• Deliver direct patient care in accordance with care plans and facility protocols.
• Monitor and document patient conditions, reporting any changes promptly.
• Assist with activities of daily living, including bathing, dressing, and mobility.
• Maintain a safe and clean environment, adhering to infection control procedures.
• Communicate effectively with patients, families, and healthcare team members.
• Comply with facility policies on COVID-19 vaccination and identification badge requirements.

Qualifications:

• Valid CNA certification and active state licensure.
• Demonstrate compassionate patient care and strong communication skills.
• Ability to work effectively in a team-oriented environment.
• Proven ability to follow detailed care plans and directives.
• Must bring valid identification and adhere to vaccine policies.
• Prior experience in LTC/SNF settings preferred.
