Work Environment
This position will be performed remotely, allowing for flexibility in your work hours while promoting a balanced work-life environment. Regular interaction with interdisciplinary teams will take place via virtual communication platforms to ensure that collaboration and care delivery are seamless. You will have the opportunity to conduct home assessments and meet with clients and families to provide guidance and support, adapting your approach based on the unique circumstances presented.
Key Responsibilities
- Perform comprehensive assessments of seniors’ psychosocial needs, identifying emotional, mental, and social factors affecting their well-being.
- Develop and implement personalized care plans in collaboration with clients and families, ensuring that specific goals and preferences are met.
- Provide counseling and emotional support, guiding clients and their families through transitions such as end-of-life care, relocation, and health changes.
- Actively advocate for clients' rights, assisting them in accessing community resources, health services, and social support.
- Work closely with a multidisciplinary team—including nurses, healthcare providers, and caregivers—to coordinate care and services effectively.
- Conduct regular virtual and in-person visits to monitor clients' conditions and safety, addressing concerns that arise promptly.
- Facilitate communication and conflict resolution among family members involved in the care of seniors, ensuring a supportive environment.
- Educate clients and their families about available resources, support groups, and programs to enhance engagement and overall quality of life.
- Maintain accurate documentation of assessments, care plans, and interactions in compliance with professional and organizational standards.
- Stay informed about the latest policies, practices, and regulations within social work, ensuring ethical compliance.
Patient Population / Client Focus
This role primarily focuses on providing support to seniors in hospice care, often dealing with complex emotional and psychosocial challenges. Your work will impact not just the individuals but also their families, helping them navigate difficult decisions and transitions during sensitive times. You will cater to diverse backgrounds and needs, emphasizing personalized care approaches that resonate with individual client values and circumstances.
Qualifications
- A Master’s Degree in Social Work from an accredited program, demonstrating advanced knowledge and skills in the field.
- Current licensing as a Licensed Master Social Worker (LMSW) in Colorado is required to practice in this role.
- A minimum of two years of relevant experience in social work, ideally within a geriatric or hospice environment.
- Proven ability to assess and respond effectively to the psychosocial needs of seniors and their families.
- Strong interpersonal skills with a compassionate approach that builds rapport with clients and families.
- Proficiency in utilizing electronic health record (EHR) systems and other technology platforms for documentation and communication.
Preferred Experience
- Experience in interdisciplinary team collaboration, especially in a hospice or palliative care setting, is highly desirable.
- Knowledge of local community resources and support services available for seniors and their families.
- Engagement in continuous professional development, including workshops and additional certifications in geriatric care or social work ethics.
Compensation and Benefits
The salary range for this position is competitive, offering an annual compensation between $60,000 and $100,000, commensurate with experience and qualifications. Additionally, we provide a comprehensive benefits package that includes:
