About the Role: We are seeking a compassionate and skilled CNA to join our dedicated team. As a CNA at Xenia Health and Rehab, you will be responsible for providing high-quality care to our residents, including: Assisting with daily living activities, such as bathing, dressing, and grooming Monitoring vital signs and reporting changes to nursing staff Assisting with med passes, transfers, and ambulating patients Maintaining accurate records and documentation Collaborating with nursing staff to develop and implement individualized care plans Responsibilities: Provide direct care to residents, including bathing, dressing, grooming, and incontinent care Assist with mealtime, including feeding, serving, and cleaning up Perform vital signs and measure intake/output as needed Assist with toileting, bath room care, and incontinence care Participate in resident care conferences and develop individualized care plans Maintain accurate and up-to-date records and documentation Requirements: Current Certified Nursing Assistant (CNA) certification High school diploma or equivalent required Ability to lift, transfer, and reposition residents with safety and dignity Ability to work effectively as part of a team and communicate effectively with residents, families, and other healthcare professionals Ability to maintain confidentiality and handle sensitive information with discretion Why Join Our Team: Join a team that truly cares : Our dedicated staff is committed to delivering exceptional care and service to our residents.
