Infusion Pharmacist

We are seeking an experienced Infusion Pharmacist to support home care patients. This role is responsible for safely dispensing medications, evaluating drug therapies, and preparing compounded/sterile medications.

Schedule: Monday–Friday, 9:00 AM–4:00 PM

Responsibilities

• Review, verify, and dispense prescription medications.
• Compound and prepare infused and sterile medications following proper procedures.
• Evaluate and monitor drug therapies for home care patients.
• Counsel patients on medication use, dosage, storage, side effects, and drug interactions.
• Identify medication-related problems and communicate recommendations to physicians and other healthcare providers.
• Maintain accurate and confidential patient records and pharmacy documentation.
• Maintain appropriate inventory of medications and medical supplies.
• Supervise and support pharmacy technicians and other pharmacy staff.
• Collaborate with physicians, nurses, pharmacists, pharmacy interns, and other healthcare professionals.
• Ensure all pharmacy operations comply with applicable laws, regulations, and safety standards.

Requirements

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred
• Active Registered Pharmacist license in the state(s) of practice
• Experience with infusion pharmacy and compounding infused drugs strongly preferred
• Experience evaluating and monitoring drug therapies for home care patients
• Strong organizational and time-management skills
• Excellent verbal and written communication skills
• Professional organization membership is a plus
