This role is ideal for a COTA who is passionate about helping residents maximize independence and quality of life in a skilled nursing and rehab setting.General PurposeThe Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist in providing patient-centered therapy services, carrying out treatment plans, and documenting resident progress in accordance with physician orders, state practice acts, and facility policies.Essential Duties and ResponsibilitiesTreat residents as directed by the Occupational Therapist and physician-approved plan of careProvide direct patient care to improve functional independence and quality of lifeRecord daily treatment notes and weekly progress notes in accordance with OT Board and payer requirementsCommunicate resident progress, challenges, and plans with the rehab and interdisciplinary teamsAssist with discharge planning and caregiver educationInstruct residents, families, and nursing staff on maintenance programs and safety techniquesAssist nursing department with training of Restorative Aides as neededParticipate in rehab meetings, care conferences, and in-services as assignedAssist with cleaning, maintenance, and organization of the therapy treatment areaReport equipment issues promptly to ensure safe operationMaintain positive, professional relationships with residents, families, and staffSupervisory ResponsibilitiesThis position has no supervisory responsibilitiesQualificationsEducation & ExperienceGraduate of an accredited Occupational Therapy Assistant programSkilled nursing or geriatric experience preferred but not requiredLicensure & CertificationsCurrent, active California Certified Occupational Therapy Assistant (COTA) license in good standingMust meet all continuing education requirements per state regulationsSkillsProficient in electronic documentation systems (Casamba and PointClickCare preferred)Strong communication, time management, and clinical skillsAbility to work independently and as part of a collaborative teamPhysical DemandsFrequent standing, walking, reaching, pushing, and pullingAbility to lift and move up to 50 pounds frequentlyOccasional stooping, kneeling, crouching, or balancingVisual abilities include close, distance, color, peripheral, and depth perceptionReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsWork EnvironmentSkilled nursing and rehabilitation settingNoise level typically low to moderateOne primary work locationWe are an equal opportunity employer.
