Part-Time RN Case ManagerLooking to hire a part-time RN Case Manager to join our growing home care services who is experienced in nursing, with emphasis on community health education/experience to meet the needs of individuals and families within their home and communities.Job Functions:Complete initial home health assessment of patient and family to determine home care needs.Conducts home visits as assigned, assessing the patients condition, progress toward goals, providing patients care, teaching and other skilled interventionsSupervisory visitsSupervising LPN and Home Health AidesInitiates the plan of care and makes necessary revisions as patient status and needs change.Counsels the patient and family in meeting nursing and related needsProvide health care instructions to the patient as appropriate per assessment and plan of carePrepare clinical notes and updatesClinical Record ReviewTake call in a call rotation.Must have 1 year of nursing experience, reliable transportation.Acute Nursing is an EOE.We offer competitive wages
