RN Care Coordinator

Kaiser Permanente is seeking an RN Care Coordinator to support inpatient members using advanced information systems and collaborative workflows. In this role, you will coordinate and monitor care from admission through discharge, using digital tools and EMR systems to track progress, close care gaps, and support safe transitions. You will partner with physicians, therapists, social workers, and outpatient teams to design and execute evidence-based care plans while educating patients and families. Drawing on data from modern care platforms, you’ll identify opportunities to improve quality, reduce readmissions, and streamline communication across the continuum of care, all within a collaborative, innovation-focused culture.
