Discharge & Care Coordination Specialist

University of Utah Health is seeking a discharge coordination specialist to implement patient discharge plans and support care coordination under the supervision of a nurse or social worker. The role interfaces with insurers, home care agencies, facilities, and equipment providers to ensure smooth transitions for patients.
