PT - Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. This role includes serving as an educator for patients, families, students, coworkers, and oneself while performing other professional duties associated with patient care and program delivery. Commitment to patient and employee safety is a priority. Responsibilities Complete comprehensive evaluations and develop suitable treatment plans for patients. Document medical records clearly, concisely, and accurately, in compliance with state, federal, and guideline standards. Ensure care is provided under signed, valid physician orders and within insurance approval. Participate in student programs as assigned, providing a positive learning environment. Develop a personalized professional growth plan and pursue continuing education and professional activity involvement. Maintain adherence to the discipline-specific State Practice Act and complete all state-mandated continuing education requirements. Demonstrate awareness of organizational performance improvement initiatives. Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited school. Minimum of 1 year of outpatient clinical experience. CPR Certification. Active and applicable license for the state where work will be performed. Schedule This position operates on a 5x8.0 schedule.
